seeking a dedicated and skilled Physical Therapist to join our
team. The ideal candidate will possess a strong background in
patient care and rehabilitation, demonstrating expertise in various
therapeutic techniques. This role involves working with patients
across different settings, including inpatient, outpatient, and
home health environments. The Physical Therapist will be
responsible for assessing patient needs, developing individualized
care plans, and implementing effective treatment strategies to
enhance mobility and overall physical function. Duties Conduct
thorough assessments of patients to determine their physical
therapy needs. Develop and implement personalized care plans that
address individual patient goals. Utilize various therapeutic
techniques including trigger point therapy, physiotherapy, and
orthopedic interventions. Provide patient education on exercises
and self-care strategies to promote recovery. Collaborate with
other healthcare professionals to ensure comprehensive patient
care. Document patient progress using EMR systems and maintain
accurate medical records. Engage in discharge planning to
facilitate smooth transitions from inpatient to outpatient or home
health care. Work with pediatric, geriatric, and acute care
populations as needed. Maintain knowledge of medical terminology,
anatomy, and physiology relevant to physical therapy practice.
Requirements: Valid state license to practice as a Physical
Therapist. Strong knowledge of anatomy and physiology as it relates
to rehabilitation. Experience in home & community care settings is
preferred. Familiarity with EMR systems for documentation purposes.
Previous experience in inpatient hospital settings or outpatient
clinics is advantageous. Ability to develop effective care plans
tailored to diverse patient populations including pediatrics and
geriatrics. Excellent communication skills for interacting with
patients, families, and interdisciplinary teams. Commitment to
