We want you to apply!Job Summary and Qualifications The medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. \n What qualifications you will need:Basic Cardiac Life Support must be obtained within 30 days of employment start dateNonviolent Crisis Intervention must be obtained within 30 days of employment start dateMasters Degree in social work1+ years of experience This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved). Lawnwood Hospital provides interventional and therapeutic care, including open-heart surgery.
