Job Description:


The Registered Nurse is responsible for the administration of direct and indirect individual care for the Program using the nursing process and nursing judgments based on scientific principles. The R.N. is guided by the Nurse Practitioner Act and established policies and procedures for the unit.


Valid and current Florida Registered Nurse License.

Maintains current CPR certification.


Must be in good physical condition with no restrictions of movement.

Must be able to apply Therapeutic Effective Non-Violent Crisis Intervention Techniques without limitations.


1.0 Observes and assesses client's course of treatment while in residence.

1.1 Nursing assessments are performed in accordance with the agency's protocol for conducting nursing assessments and its procedures relative to frequency and timeliness for the accordance of this activity.

1.2 Assessment data is organized, accurate, and complete.

1.3 Documents clients progress according to identified needs and goals of the treatment plan.

1.4 Administers nursing care/treatment according to Agency protocol and procedures in accordance with accepted nursing care standards.

1.5 Attend and participates in daily treatment team meetings as appropriate.

1.6 Recognizes and reports any signs of aggressive behavior utilizing techniques of non-violent crisis intervention.

1.7 All clients on special precautions are monitored according to unit protocol and procedure.

1.8 For ASU and CCSU only, utilizes individual seclusion and/or restraints in accordance with unit protocol and procedure, as a last resort.

1.9 Receives and interprets change of shift report.

1.10 Reviews each client, evaluating plan of care for those clients assigned.

2.0 Administers physician ordered medications to clients and ensures and accepts responsibility for security of medication at all times

2.1 Demonstrates the ability to administer medication and/or oversee self-administration in accordance with unit policy and procedure.

2.2 Documents medication education.

2.3 Accurately counts, administers and charts all medications given according to established procedure.

2.4 Verifies original medication order by client name, medication, dosage, time and route of administration.

2.5 Administers all PRN medications according to appropriate indications and verifies with original order.

2.6 Administers all scheduled medications within 60 minutes of time ordered.

2.7 Monitors and documents responses/reactions to medication in accordance with unit protocol and procedure.

2.8 Documents all medication variances and reports same to Program Manager.

2.9 Medication count is conducted at the beginning and end of each shift, as assigned.

2.10 Medication area is secure and organized, neat, and orderly at all times.

2.11 Ensures charts are assembled properly.

3.0 Prepares written documentation for all services rendered.

3.1 Written documentation for services rendered is performed in accordance with the Agency's standards for service documentation in the client's medical record.

3.2 Accurately documents observations, procedures and care provided during the client's course of treatment.

3.3 All charting is signed with first initial, last name and title 100% of the time.

3.4 Documentation is neat, legible and in accordance with accepted charting standards.

3.5 Documents individual progress according to identified needs and goals of the treatment plan.

4.0 Provides general direction and oversight to RSTs on assigned shift.

4.1 Ensures that all RSTs have a clear definition of their work responsibilities.

4.2 Consistently assigns tasks to RSTs in accordance with their personal abilities and limitations.

4.3 Consistently demonstrates the ability to train and develop RSTs in their responsibilities.

4.4 Ensures that all RSTs routinely utilize policy and procedure manuals and other available reference materials as necessary to consistently ensure quality of service provided.

4.5 Consistently motivates RSTs in a positive manner; develops teamwork in accepting and practicing the policies and procedures of the Agency.

4.6 Provides direction as necessary; always leads by professional example.

4.7 Ensures individual admissions are performed according to unit protocol and procedure.

4.8 Ensures all staff remain awake and alert.

5.0 Demonstrates a high degree of dependability.

5.1 Maintains a good attendance record.

5.2 Observes working hours.

5.3 Notifies supervisor of any absence 4 hours prior to beginning of shift.

5.4 Reports promptly for shift report.

5.5 Call-ins and unscheduled absences are in accordance with agency policy.

5.6 Supervisor is always notified and approval granted prior to leaving assigned work area.

5.7 Shows flexibility and is responsive to requests from supervisor.

5.8 Works independently with minimal supervision.

5.9 Exhibits characteristics of good workmanship and integrity on the job.

5.10 Work time is highly productive.

6.0 Establishes and maintains effective and positive working relationships with clients, co-workers and others.

6.1 Readily develops and establishes therapeutic relationships with clients.

6.2 Effectively interacts with client's family and significant others.

6.3 Is courteous to clients, staff and all others with whom contact is made.

6.4 Demonstrates tact at the appropriate time.

7.3 Disposes of medication, Biohazardous materials and sharps appropriately and in a timely manner.

7.4 Demonstrates awareness of malfunctioning equipment and supply shortages.

7.5 Reports potential hazards and issues work orders for correction.

7.6 Demonstrates appropriate use of Unusual Incident Reports in accordance with the Agency's Unusual Incident Reporting procedures.

8.0 Establishes and creates a therapeutic milieu on assigned shift.

8.1 Utilizes appropriate voice tone, volume and word choice when dealing with escalating individuals.

8.2 Demonstrates good problem solving techniques.

8.3 Ensures groups are held as scheduled and directs that additional groups be held as clinically indicated.

8.4 Provides feedback to RSTs and CAs regarding therapeutic interventions offering suggestions as needed.

8.5 Demonstrates ability to assess when early interventions are needed and either provides the intervention or directs other staff.

8.6 Acts as a role model in the use of de-escalation techniques and the teaching of adaptive coping skills with individuals.

9.0 Presents a positive personal image.

9.1 Dresses according to departmental dress code.

9.2 Identification badge is worn at all times.

9.3 Maintains a professional demeanor.
