The ADVANCED CLINICAL PHARMACIST-MEDICATION RECONCILIATION is
responsible for the safe, effective, evidence-based, and
cost-effective selection and utilization of medications for the
patients in which they serve. The clinical pharmacist uses advanced
knowledge, skills, and professional judgment in all aspects of his
or her work. The clinical pharmacist serves as a patient advocate
when planning and implementing medication therapy within an
interdisciplinary rounding team, collaborating with other
healthcare professionals in a knowledgeable and professional
* Home Medication List Review and Medication Reconciliation
* Provides leadership in managing patient-centered medication
reconciliation on admission and discharge.
* Identifies and resolves drug related problem
* Performs discharge counseling for patients at risk of readmission
for patients and their caregivers managing medications in the home
or equivalent setting
* Analyzes data to demonstrate the success of the pharmacy led
medication reconciliation safety program.
EDUCATION & EXPERIENCE
* B.S. or Pharm D. degree in Pharmacy
* Graduate of an accredited residency program, preferred
* (PGY1) Pharmacy Practice Residency, preferred, preferred
* (PGY2) Specialized Residency in area of specialty, preferred
* 3-5 years hospital pharmacy experience with supervisory
LICENSES & CERTIFICATIONS
* Maintains current pharmacist licensure, required
* National Provider Identifier (NPI), required
