Connected for Kids Program provides services for children and
families in the child welfare system, to ensure a community without
abuse, neglect and abandonment. Based out of Fort Pierce, FL, this
position is responsible for providing safety assessment and
coordination of services for children and families referred by the
Department of Children and Families due to abuse, neglect, or
abandonment by caregivers. Additional responsibilites of the
position include: + Conduct child safety assessments. + Assess
client needs and develop service plans + Coordinate the delivery of
services, plan referrals and linkages to clients and families +
Monitor service plan progress and evaluate reports from multiple
service providers + Develop and maintain case and program
documentation according to contract and CCKids standards
(assessments, treatment/service plans, progress notes, termination
summaries, etc.) + Prepare case/status reports and make
recommendations/testify at courts or other prescribed body
regarding reunification, termination of parental rights or other
permanency plans + Conduct diligent searches as required by
Dependency laws + Coordinate services with other professionals and
paraprofessionals + Conduct community outreach as needed + Provide
specialized services that may include detainment, interstate
compact, post placement supervision, relative/non-relative
caregiver home studies, court testimony, termination of parental
rights and judicial review staffing + Provide support to children,
caregivers and family members to assure stable placement and timely
achievement of permanency + Assist family/caregivers in accessing
subsidy, medical coverage and other needed services for children +
Coordinate cases with supervisor, team members, providers, DCF and
CLS attorneys as needed + Facilitate placement and independent
living + May remove and place children + May facilitate mediation
staffing for reunification, TPR and detainment + Arrange or provide
transportation of clients when needed + Arrange for drug/alcohol
screens as necessary + Testify in dependency court proceedings and
provide court work to CLS attorneys + Provide on call support as
required or scheduled + Maintain required child welfare
certification + Other assigned duties as required _Keywords: child
welfare, case management, dependency, human services, social work,
social services, social worker, child advocate, family advocate,
BSW, MSW, CWCM, CPI_ **Qualifications** **Education:** + Bachelor's
degree _required_ from an accredited university in social work or
related area of study. Related degrees include: criminology,
juvenile justice, psychology, sociology, counseling, special
education, education, human development, child development, family
development, marriage and family therapy, or nursing. Will also
consider human services and criminal justice with DCF
authorization. **Other Requirements:** + Upon hire all Case
Managers are required to obtain certification as a Child Protection
Professional with 12 months. CWCM a plus. + Valid FL State Issued
Driver's License + Work schedule hours will be a core Monday -
Friday, dayshift; however flexibility to work outside of regular
business hours is required including evenings and weekends due to
